Modeling an odd part

I am attempting to model this part made of 1/8" steel. The part is 36"x3".View attachment 5940View attachment 5941

I have relief cuts made, then the part is bent. When I try to model this in solidworks I am having one heck of a time. I cannot put the V cuts in before bending, because it gives me an error (I am using sheet metal).

I need a good drawing of the start and finish of this job to give to the guys on the shop floor. Any ideas?
 
Can the part be made up from 6 individual pieces, with the required angles, then welded up?

Here's a "kludge", maybe. Make your part 1/8" x 1/8" x 36" thick. Make the required bends (no relief cuts, zero bend radius). Make "tabs" on the straight sections 2-7/8" long. Shape ends of tabs to fit.

I'm not near a SW installation to verify, but it might work.
