Associate/Relate/Group GTOLs to Dims

How can you group a GTOL to a dimension? And more specifically to a diameter dimension, so the GTOL is centered under the diameter? Right now the only thing I've come up with is placing the GTOL under the dimension (unrelated) and modifying the dimension usingRETURN/ENTER to put a couple of extra blank lines below the dimension text (to make space above the leader line.)
