When you are creating a form in sheetmetal, Pro will prompt you to select the boundary surface and then the seed surface after you have placed the form on your sheetmetal part.
There is no menu per se. You will see the boundary and seed elements in the definition window (upper right of your screen) when you create your form.
Its in setup, it also prompts you to select it to unbend if you have not previously set it. I generally set the reference surface very early in the design, just a habit I got into.
Oops, I skimmed your question and made assumptions, big mistake.
The bounding surface must fully encircle the form feature or else it will fail when you go to use it and the seed surface should be a round. You can redefine the form just like any other feature.
