multiple mapkeys

How to add a mapkeys like the one shown below? I can able to customize single mapkey in the screen. But i want to know the procedure toadd multiple mapkeys one over another as shown in the below example.

hi,
insert new flyout manu first as shown below...then you can drag mapkeys as uou posted to that new flyout menu...
